Rates
 Initial Session:
 $120: Individual Counseling/Psychotherapy (up to 50 minutes)

 $120: Career or Life Counseling (up to 50 minutes)

 $95: NeurOptimal neurofeedback relaxation and brain training session (up to 50 minutes total appointment time)

Subsequent Sessions:

 $95: Individual Counseling/Therapy (up to 50-minutes)

 $95: Career or Life Counseling (up to 50-minutes)

 $75: NeurOptimal neurofeedback relaxation and brain training/optimizing session (takes approx 45-50 minutes)

Insurance

   Services (except Neurofeedback) may be covered in full or in part by your health insurance or employee benefit plan.

   Please check your coverage carefully by asking the following questions:

• Do I have mental health benefits?
• What is my deductible and has it been met?
• How many sessions per calendar year does my plan cover?
• How much does my plan cover for an out-of-network provider?
• How much does my plan cover for an in-network provider?
• Do I need an authorization or confirmation number?
• Is approval required from my primary care physician?

    Medicare Provider? YES, Medicare Florida (Original Medicare Part-B, with or without a Supplemental plan).

    United Health Care: YES, In-Network Provider.

    Blue Cross/Blue Shield: I am a registered Provider (Out of Network) and can provide you with a bill for your submission to BCBS

Reduced Fee
Reduced fee services may be available, on a needs-based, and limited basis.

Payment
Cash, check and all major credit cards accepted.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the appointment.
